ENROLLMENT DOCUMENTATION
To enroll your child please complete all requirements below. If you do not have access to a printer, you can complete the enrollment forms at our school.
Under the McKinney–Vento Homeless Assistance Act the above requirements are not required at enrollment if you are in Temporary Housing and all you need to provide is a letter of residency which you can receive from your housing liaison.

- Complete NEW STUDENT ENROLLMENT/REGISTRATION FORMS (LINK)
- Provide ALL supporting documentation on Registration Checklist (link)
- Proof of Age - Child's Birth Certificate or Passport
- Two Proofs of Residency - Lease and one other document from the Registration Checklist (link)
- Immunizations - For more information visit NYC DOE Immunizations (link). For other health services visit NYC DOE Health and Wellness (link)
- Health Form - The school requires your child's physical every year, CHILD & ADOLESCENT HEALTH EXAMANATION FORM (link)
